Birth Certificate and Social Security Number

New parents will be contacted the day after baby’s birth to gather the required information that we will forward to the Office of Vital Records in New Orleans. After the application is processed, that office will mail the birth certificate/social security number to the parent at home. Though we do give mom a commemorative certificate of baby’s birth, the hospital does not provide an official, legal copy of the birth certificate. Patient Medical Record Request Process for Lafayette General Medical Center and Lafayette General Surgical Hospital

To request medical records you may visit the Health Information Management (HIM) Department located on the Mezzanine floor within Lafayette General Medical Center. A Release of Information Clerk will be available to process your request between the hours of 8:00 am and 4:00 pm, Monday through Friday. Feel free to call a Release of Information Clerk at 289-7771 for questions.

We are extremely committed to privacy, therefore, you must come to HIMin person in order for you to sign an authorization and present proper identification. We can not release medical record information over the phone. To save time, you may download the Medical Records Request Authorization Form, complete fully and bring it to the medical records office.

The Health Information Management Department within Lafayette General Medical Center handles medical record requests for Lafayette General Surgical Hospital and Lafayette General Imaging. We do NOT issue certified copies of birth certificates (see above). If your request requires Radiology film please contact Radiology Department at 289-7970.

Patient Medical Record Request Process for University Hospital & Clinics

To request medical records, you may either visit the Health Information Department located on the first floor at University Hospital and Clinics or you may have your provider fax an authorization for release of information to 261-6077.  

A release of information clerk will be available to process your request for records between the hours of 8 am and 4 pm. For further information regarding releasing your health records, feel free to call a release of information clerk at 261-6191.

Should you need a copy of a CD from radiology, you must call UHC’s radiology department at 261-6244 or 261-6251. The Health Information Department at University Hospital and Clinics can only release records for UHC. Download the UHC Medical Record Request form.

Patient Medical Record Request Process for St. Martin Hospital

To request medical records you may present to the outpatient registration desk. Personnel from the Medical Records Department will be available to process your request between the hours of 7:30 am and 4:00 pm, Monday through Friday. Feel free to call the Medical Records Department at 507-1215 or 507-1218 for questions.

We are extremely committed to privacy, therefore, you must come in person in order for you to sign an authorization and present proper identification. We cannot release medical record information over the phone.

Patient Medical Records Request Process for Abrom Kaplan Memorial Hospital

To request medical records you may visit the Outpatient Department located in the front lobby of the hospital.

Our Outpatient/Admit Clerk will be available to accept your request for medical records between the hours of 7:30 am and 4:00 pm, Monday – Friday. Once a request is received, our outpatient clerk will call our Medical Records Department to obtain your health information. Please feel free to call our Medical Records Department at 643-5301 with any questions or concerns.
