Lafayette General Medical Center

LGMC provides Registration Services in the main lobby on 1st floor; hours of operation are Monday-Friday, 5:00 am to 5:00 pm. In order to expedite the registration process, we ask that you pre-register at least one week in advance of your hospital services to allow time for insurance verification and pre-certification, when required. For your convenience, you may pre-register by calling (337) 289-7162, visit our registration area in the hospital main lobby, or complete the online pre-registration form.

If your doctor has ordered diagnostic testing prior to your procedure, please visit LGMC Diagnostics, located at 1211 Coolidge Blvd. (within the Burdin Riehl building) on the 2nd floor. Hours for LGMC Diagnostics are Monday through Friday, 6 a.m. to 5 p.m. and they can be reached at (337) 289-8000 to answer any questions you might have. The staff can take care of your registration and diagnostic testing needs. Free valet parking is offered at the front of the building.

If you are having pre-procedure testing performed, you will still be required to check-in with LGMC registration on the date of your actual procedure, as necessary signatures and copies of insurance cards will be obtained at that time.

Lafayette General Surgical Hospital

LGSH provides Registration Services in the main lobby on the 1st floor; hours of operation are Monday-Thursday, 7:30 am to 4:00 pm and Friday, 7:30 am to 12:00 pm. Registration is on a first-come, first-serve basis. If you have any questions, please call (337) 289-8085. If you need to reach the nurses station, please call (337) 289-8095. 

University Hospital & Clinics

University Hospital & Clinics provides Registration Services on the 7th floor; hours of operation are Monday-Friday, 6:00 am to 4:00 pm. The Emergency Room is open 24 hours a day, 7 days a week. If you have any questions, please call (337) 261-6000. 

St. Martin Hospital

St. Martin Hospital provides Registration Services in the main lobby of the hospital. Hours of operation are Monday-Friday, 6:30 am to 7:00 pm. In order to expedite the registration process, we ask that you pre-register at least one week in advance of your hospital services to allow time for insurance verification and pre-certification, when required. For your convenience, you may pre-register, for services listed below, by calling (337) 507-1209, Monday-Friday between the hours of 7:30 am to 4:00 pm. In order to schedule a procedure, a physician order is required.

If your doctor has ordered diagnostic testing prior to your procedure, please visit the Registration area located in the Outpatient Services wing of the hospital. The staff can take care of your testing and pre-registration needs. If your doctor has given admit orders, please bring these with you. You will still be required to check-in with registration on the date of your actual procedure, as necessary signatures and copies of insurance cards will be obtained at that time. Abrom Kaplan Memorial Hospital

Abrom Kaplan Memorial Hospital provides Registration Services in the main lobby of the hospital. Hours of are Monday-Friday, 7:00 am to 4:00 pm. From 4:00 pm to 6:00 pm, Registration Services are located at the Switchboard Operator’s Office adjacent to our Emergency Room. In order to expedite the registration process, we ask that you pre-register at least one week in advance of your hospital services to allow time for insurance verification and pre-certification.

For your convenience you may pre-register by calling (337) 643-5285, Monday-Friday between the hours of 7:00 am to 4:00 pm. Please remember that in order to schedule a procedure a physician order is required. If your physician has ordered diagnostic testing prior to your procedure, please visit the Registration area located in Outpatient Services near the Hospital Lobby.

Our staff can take care of your testing and pre-registration needs. A physician's order is required on procedures. Please verify whether your insurance carrier covers the service to be provided, as each plan is different.
