Patient Medical Record Request Process

To request medical records you may visit the Health Information Management (HIM) Department located on the Mezzanine floor within Lafayette General Medical Center.  A Release of Information Clerk will be available to process your request between the hours of 8:00 am and 4:00 pm, Monday through Friday.  Feel free to call a Release of Information Clerk at (337) 289-7142 for questions. 

We are extremely committed to privacy, therefore, you must come to HIM in person in order for you to sign an authorization and present proper identification.  We can not release medical record information over the phone.

You may download the authorization form, below, and bring it with you already complete.

The Health Information Management Department within LGMC handles medical record requests for Lafayette General Surgical Hospital and Lafayette General Imaging.  We do NOT issue certified copies of birth certificates (see below).

Birth Certificate and Social Security Number

New parents at The Pavilion will be contacted the day after baby's birth to gather the required information that we will forward to the Office of Vital Records in New Orleans.  After the application is processed, that office will mail the birth certificate/social security number to the parent at home.  Though we do give mom a commemorative certificate of baby's birth, the hospital does not provide an official, legal copy of the birth certificate.  To check on the status of baby's certified birth certificate, contact the Office of Vital Records at 504-219-4500.
